If you are new to Friesz Family & Cosmetic Dentistry in St. Cloud, MN, we have all the dental forms you need right here on this page. We have tried to make the registration process seamless and easy with our online patient information forms, and our experienced staff is ready to answer your questions and address any concerns you may have about your visit. Whether you’re coming to see us for teeth whitening or to begin the implant process, you’ll need to complete these forms.
Please fill out all forms possible prior to your appointment to help keep the process moving along quickly. Being able to access these forms before you come to the office will save you time and help our staff get all the information needed to process any insurance you may have, as well as get your initial information entered into our system. Forms and Documents to Bring
We provide medical history, registration, and privacy forms online for both new and returning patients to view, print, fill out, and bring into the office, expediting the registration and information update process. Please fill out all fields on our forms so we can provide proper diagnoses and treatment.
Although your form may completely list your insurance coverage, we request that you bring in your insurance coverage card along with proof of ID on your first visit so we can verify your coverage for you or your dependents. Changes in your insurance coverage require that you notify us ahead of your next appointment and provide us with your new insurance card on your next visit to verify your change in coverage for you or your dependents.
REGISTRATION FORM
This registration form should be filled out fully prior to your first visit. It has all the basic information we need to get you into our database. Since we are a family dentistry practice, we also need basic information for any children who are coming to our office as well.
MEDICAL HISTORY
Our dental practice needs to be aware of your complete medical history to provide you with safe, effective treatment, whether you’re coming in for a dental implant or a teeth cleaning. Please fill out this form completely and let us know if any information needs to be updated down the line. Be sure to list any medications you are currently taking.
RELEASE FORM
This release form gives consent to speak with patients regarding health status, including diagnosis, treatment options and plans, and payment for health services.
PATIENT CONFIDENTIALITY
We value the confidentiality of your patient information and advise that new patients bring their registration forms in with them as sending your health information by email may put your information at risk of being intercepted. Request Medical Records
As a patient with us, you have full access to your medical records. Simply fill out a medical records request form and a member of our staff will process your request and release a physical copy of your information to you. Please make sure to secure any physical copies of your information in a safe place or dispose of your records safely should you no longer need them. Our staff will gladly and safely dispose of any extra copies of your medical records should you want or need to get rid of them for any reason.
